Have everyone here, proof of citizenship, proper type of identification, and your checkbook.

***A Driver’s License that states it is NOT FOR FEDERAL ID is not a valid ID for passport applications. You'll need a 2nd photo ID. Driver’s Permits also need to be accompanied by a school ID.

Passport Renewal

  • Completed Application DS-82
  • Expired Passport
  • Recent Photo $15.00 (here)
  • Payment to: US Dept. of State - $130.00 (Check)

New Passport - Adults

  • Completed Application: DS-11
  • Certified Birth Certificate
  • Photo Identification
  • Recent Photo: $15.00(here)
  • Payment to: US Dept. of State  - $130.00 (Check)
  • Payment to: Town of Stowe - $35.00 + Photo fee if taken here (Cash, Check or Credit)
  • *May need DS-64 (lost passport) or DS-5504 (name change)

New Passport - Children

  • Completed Application DS-11
  • Certified Birth Certificate
  • Recent Photo $15.00 (here)
  • Photo Identification Both Parents must be present
  • If 1 parent is unavailable: Complete DS-3053 Form
  • Payment to: US Dept. of State - $130.00 for 16 years & older, or $100.00 for 15 years & younger (Check)
  • Payment to: Town of Stowe - $35.00 + Photo fee if taken here (Cash, Check or Credit)
